In June 1982, the engines of British Airways Flight 009 failed after it flew through volcanic ash caused by an eruption and the plane began falling from the sky. Minutes before it would have crashed into the sea the engines roared back to life - narrowly avoiding a catastrophe. The investigation looks at the lessons learned from the incident, which changed the way pilots were trained
